List of documents required for execution of a pay card

• The client’s ID.


• An application for opening an account and issuing a bank pay card, executed in accordance with the established form (hereinafter — "Application for execution of a Card), in 2 copies.


• For underage customers (aged between 14 and 18) — a written authorisation from one of his/her parents, adoptive parents, custodians (to provide for the possibility of an overdraft). An application form can be obtained at any of the Bank’s offices.


• Payment of the account servicing fee, including the insurance bonus (if required).


• Payment of the initial deposit (if required).


• Placement of the security deposit amount (if required).


• An application form executed for the purposes of the Agreement on a full package of services, in 2 copies (if necessary).
