To enter into an individual complex service agreement with Bank of Moscow it will be enough to visit any office of the Bank with a passport or any other ID and to complete a questionnaire. (A questionnaire may be completed by yourself or with the help of the Bank’s employee).

Before completing the Application, please read carefully the "the Bank of Moscow Personal Comprehensive Service Contract Rules", which also include Rules for issue and use of bank payment cards of the Bank of Moscow.

If you agree with the terms and conditions of the Rules of Provision of Individual Complex Service by Bank of Moscow you can print out a Questionnaire, complete it and have the completed questionnaire with you when buying a bank product/service at any office of the Bank.

Rules of completing a questionnaire

  • Print out a questionnaire on one page (on both sides if required).
  • A questionnaire is to be completed in block letters. All points are to be completed.
  • Illegible questionnaires will not be considered.
